Secure Horizons ValuePlan: An Affordable Health Plan — Just for HealthCare Partners' Medicare-eligible patients
We are pleased to announce that we have entered into a special arrangement with Secure Horizons from PacifiCare® to provide our Medicare-eligible patients in Los Angeles County with an affordable health plan option for 2005.
Called the “Value Plan,*” this plan offers the most comprehensive benefits available in LA County from Secure Horizons. The Value Plan is available to all eligible Medicare beneficiaries who enroll for the 2005 benefit year. The Secure Horizons Medicare Advantage Value Plan includes:
- $0 monthly health plan premium.
- Unlimited brand-name prescription coverage
- Low co-pay for office visits; $5 for primary care physician; $10 for specialists
- $100 co-pay for outpatient surgery and outpatient hospital services.
- Membership in our SilverSneakers® Fitness Program including group fitness classes at participating fitness centers
- Co-payment for inpatient hospital stays
- Select hospitals are just $50 per day for days one through eight, up to a maximum of $400 for each covered stay.
- Standard hospitals are just $100 per day for days one through eight, up to a maximum of $800 for each covered stay
- A Solution for Caregivers helps those who are caring for an aging loved one. Members have unlimited toll-free access to obtain personalized research and caregiver resource referrals; in-person Geriatric Care Management services, and legal assistance for comprehensive caregiving support
- And, the Secure Horizons Medicare approved Discount Drug Card**
