Care Team Team of dedicated physicians and staff working together to provide quality care and services to defined panels of patients, creating a "small office" feeling.
Care Unit One clinician along with the Care Team support staff that rotate around the clinician.
Care Team Member (CTM) The support staff that provide various kinds of support functions within the Care Team.
CTM I Entry-level medical assistant. The CTM I welcomes patients, collects patient co-pays, and monitors the Patient Care Center (see definition below).
CTM II An experienced Medical Assistant (certified by an accredited school). The CTM II serves two roles, alternating between them. One role is "rooming" patients and performing typical MA functions (CTM Room). The other role entails handling incoming patient calls, scheduling appointments, and taking messages for the clinician (CTM Phone). Two CTM IIs rotate between these two jobs. The CTM II also registers patients and interacts regularly with the Patient Support Center. In addition, the CTM II participates in Care Team Huddles (see definition below).
Care Team Rotation The term used to indicate the rotating roles of the CTM IIs. The Care Team model provides great support for the patient, the physician, the CTM Phone and the CTM Room. Rotation both provides cross-training and allows each CTM to get to know the whole Care Team as well as the patients they care for. It also provides an opportunity to learn new skills and expand patient care talents.
Care Team Supervisor (CTS) The CTS is similar to a Nurse Manager or Supervisor, but with an expanded supervisory role. The CTS is an LVN or RN (license required). The CTS ensures proper staffing and scheduling and manages the Care Team rotations.
Patient Liaison  (PL) The PL conducts new patient orientation and helps the patient obtain the services they need from HealthCare Partners. The PL also acts as a patient advocate by assisting with any issues, complaints, or concerns a patient may have.

Prospective New Enrollee A prospective patient who is interested in HealthCare Partners' services.
Premier Appointments A system that enables patients to see their doctor whenever it is convenient, including same-day appointments.
Tickler File A filing system designed to assist the CTM Phone and CTM Room in maintaining a tracking process for the accurate delivery of treatment orders by the clinician. There are two files: a current month listing and a future month listing of orders.
